Play-In Games This Year

Could someone explain how this works this year? On bracket projections it looks like two play-in games are for two of the four 16 seeds, and two other play-in games are for two of the four 12 seeds.

Is this always the case? Sometimes it looks like there are two play-in games in one region too. Is that right?

Two games are for the last two at-large spots. This is likely on the 12-line but in theory could be #11 or #13 I believe. The other two are on the #16 line. I don't know if there are any rules about 1 being in each region or not.
